Proper Signatures
Repair shops must obtain customer authorization where required by the Policies & Procedures.
The Final Repair Account Signature Sheet (digital version, fax version, DR version) , which identifies the parts and labour required to return customer vehicle to its pre-accident condition, must be sent to MPI with every request for payment.
The Final Repair Account Signature Sheet requires the following signatures:
- Authorization to Estimate (Direct Repair version only): This signature verifies that the customer (registered owner / authorized contact) has authorized the repair shop to estimate the damage. This must be obtained prior to the shop creating an estimate.
- Authorization to Repair: This signature verifies that the customer has authorized the repair shop to repair their vehicle. This must be obtained prior to any repairs being completed on the vehicle.
- Certification of Repair: This signature confirms the customer is satisfied with the completed repairs. Payment will not be made without all signatures, unless payment is for a tear down. Tear downs require only one signature for authorization to repair.
- Shop Use Only: Once repairs are complete, the repair shop completes this section to confirm parts have been supplied and services rendered as outlined in the estimate.
Note: Repair shops cannot authorize estimates or repairs on behalf of a customer.
